 Instructions for Completing Registration Form

The Fall 2010 Registration Form is now in MS-Word format, allowing students to complete the form by typing the necessary information and printing it out, or saving it on your PC.  You can then either print the completed form out and mail it in with your payment, or, if using a credit card, you can send it to us as an email attachment or fax it in.

Returning students should list their holyapostles.edu email addresses at the top of the form in the appropriate location.  New students, as well as returning students who have not yet received a holyapostles.edu email account should list their personal email address.

There is a check box next to each course.  For the course(s) for which you wish to register, click on the check box next to that course and an X will appear in the box.

Please note that, for the courses which use lecture materials from International Catholic University, there is a drop-down list to the right of the course information which allows you to select which format you wish to receive the lectures in.  The choices are DVD, Audio CD, Both DVD and CD, or None.  The cost remains $65 per set, for both DVDs and CDs.  Fr. Mullady's Moral and Psychology course, and his new Fundamental Moral Theology course are both available in Audio CD only (no DVDs).

Credit card information (Visa or MasterCard) can be entered at the bottom of the form.
